This quest is repeatable on new games (though you only get the chest the first time) and you can kill her repeatedly for rune drops.

The Countess can be found in the Forgotten Tower Level 5, a side dungeon in the Black Marsh area in Act 1. Then continue to farm her as needed in normal to nightmare mode to create your first runewords (very powerful weapons/armor > list in #10)-Countess Rune Drops: (Always kill on /players1) Normal: Neither The Countess nor the Hellforge will ever drop an Ohm rune on any difficulty.

On Hell, the Countess can drop anything from El to Ist or Lo runes, while every rune above Ist will drop less often.

    It is estimated that the Countess has an 8% chance to drop a Key of Terror on Hell difficulty. You can rely on your mercenary to do the heavy lifting, assuming a good loadout. Her special rune drop only goes up to Ist. The Countess also has a chance to drop the Key of Terror, which is required for the Pandemonium Event. Her normal drop goes up to Lo, but she’s no more likely to drop a Lo rune than any other Single runes can drop anywhere in Diablo 2: Resurrected, starting in Normal difficulty through to Hell difficulty.
